PHI Handling by Providers and HIPAA If you are a patient under the care of a Provider, the usage and disclosure of your Protected Health Information are governed by HIPAA, in conjunction with other relevant laws, as well as the specific agreements detailed in the Customer Documents with your Provider. It is important to note that the collection, usage, disclosure, and transfer of such PHI by your Provider are further governed by the terms and conditions established between you and your Provider, in addition to their privacy practices. Any inquiries or requests concerning your PHI should be directed to your Provider. We hold no responsibility for how our Provider customers manage the PHI that we collect on their behalf, and we recommend reviewing their respective privacy policies.

4.1.3. Treatment of PHI on Our Sites Generally, our Sites are not designed to gather or retain any PHI. Consequently, the sections of this Privacy Policy that pertain to the collection of Personal Information on our Sites do not apply to PHI.
